Will Do: Works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care environment.

Requirements

Candidates must hold a Bachelor's degree in Physical Therapy or higher and possess a valid State of WI Physical Therapist license. A valid driver's license and BLS certification are also required, with preference given to those with home health or physical therapy experience.
