About the Role

Join AdvisaCare as a Per Diem/Part Time Hospice MSW Social Worker in United States. AdvisaCare Home Health and Hospice is looking for a Full time compassionate MSW (Masters of Social Work)/Volunteer Coordinator to join our team in the St. Clair County areas with our Troy location being your Parent Agency.

Key Responsibilities

  • Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
  • Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.
  • Conducts reassessments of patient/family needs and counseling as required.

Requirements

Candidates must possess an MSW degree from a CSWE-accredited school of social work and ideally have one year of experience in Home Health Care and hospice experience. A valid driver's license, car insurance, and reliable transportation are mandatory.
