About the Role
Join TGH Home Care as a Speech Therapist Home Health Visits Brandon/Valrico in United States. TGH Home Care powered by VNA of Florida provides a full array of home care services for TGH patients, and those in the community needing home care. This office continues the same level of care and experience received at Tampa General Hospital.
Key Responsibilities
- will ensure delivery of quality speech therapy services and provide case management for patients through comprehensive assessments and coordination of agency services.
- will work with family members and local healthcare professionals to implement services to meet the needs of patients and families in their homes.
- will be responsible for completing physical assessments and determine any patient home care needs while working with the care team for each patient while using VNA of Florida's case management model.
