About the Role
Join VNS Health as a Senior Social Worker – Home Care, $77K-$96K in United States. Overview Provides clinical and consultative support to Licensed Social Workers (LSWs) and Interdisciplinary Care Team Managers regarding complex social work related clinical and/or customer service issues. Provides mentoring and leadership to less experienced LSWs.
Key Responsibilities
- provide direct social work services to patients and families in collaboration with the interdisciplinary car.
- Conduct field orientation and orientation co-visits for newly hired social workers.
- Provide supplemental support, coaching, and clinical consultation as needed for both new hires and incumbent.
- Conduct annual clinical co-visits for SWs within their assigned regions.
