About the Role
Form Health, Inc. is a virtual obesity medicine clinic delivering evidence-based obesity treatment through telemedicine. They seek experienced Physician Assistants passionate about obesity medicine to provide longitudinal telehealth care.
Key Responsibilities
- Collaborate with physicians and dietitians to deliver team-based care.
- Provide evidence-based lifestyle counseling and pharmacotherapy.
- Conduct telemedicine visits.
Requirements
- Physician Assistant degree.
- Licensed to practice and prescribe legally, preferably credentialed in Illinois.
- 2+ years experience in obesity medicine or related fields.
- Excellent interpersonal skills.
