About the Role

Join Healthforce as a Travel Social Worker Case Management Job in United States. Overview Social Worker – Case Management (MSW/RN) – Everett, WA. Travel assignment in a Level II trauma center with two nearby campuses.

Key Responsibilities

Conduct psychosocial assessments and develop discharge plans for adult and geriatric patients in Med

Surg and ED

Coordinate care with the interdisciplinary team (PT, Interpretation Services, Social Services, Pharmacy, Transportation)

Document assessments and care plans in EPIC; ensure timely updates and accurate charting

Requirements

Education and licensure: MSW with WA LMSW/LCSW eligibility or WA RN license; BLS for RN track

Licensure: WA LMSW/LCSW or WA RN license as applicable

Experience: Minimum 1 year of acute care experience with discharge planning/care coordination; strong preference for case management across Med

Surg, long LOS, and ED
