About the Role
Join Healthforce as a Travel Social Worker Case Management Job in United States. Overview Social Worker – Case Management (MSW/RN) – Everett, WA. Travel assignment in a Level II trauma center with two nearby campuses.
Key Responsibilities
- Conduct psychosocial assessments and develop discharge plans for adult and geriatric patients in Med
- Surg and ED
- Coordinate care with the interdisciplinary team (PT, Interpretation Services, Social Services, Pharmacy, Transportation)
- Document assessments and care plans in EPIC; ensure timely updates and accurate charting
Requirements
- Education and licensure: MSW with WA LMSW/LCSW eligibility or WA RN license; BLS for RN track
- Licensure: WA LMSW/LCSW or WA RN license as applicable
- Experience: Minimum 1 year of acute care experience with discharge planning/care coordination; strong preference for case management across Med
- Surg, long LOS, and ED
