Key Responsibilities
- Provide comprehensive Physical Therapy evaluations and treatments, under referral from the attending physician, to overcome specific losses of function, impaired safety, and pain.
- Open cases, completing Oasis assessment, if on intake the patient is assessed as having no nursing needs.
- Evaluate the home environment and make appropriate recommendations.
- Effectively communicate with the referring physician and nursing staff concerning changes in the patient’s condition, and necessary changes to the initial Plan of Care due to such changes.
Requirements
- Degree in Physical Therapy from an APTA approved and accredited college or university.
- Current licensure as a Physical Therapist in the Commonwealth of Virginia.
- CPR certification (adult only)
- Passes an assessment of competencies prior to providing patient care, with any unmet areas addressed through education and mentoring prior to participation in patient evaluations and care.
