Key Responsibilities

• implement and monitor care plans, communicate with prescribers to obtain orders, report outcomes, make therape.

• implement or modify existing treatment plans Act as a subject matter expert and clinical knowledge resource to.

• Coordinate activities with local or national pharmacy team members, including pharmacists, pharmacy technicians.

• coordinate to ensure an efficient customer focused approach and optimal clinical patient outcomes Documentation.
