Key Responsibilities

• home physical therapy evaluations and treatments in accordance with established standards.

• Develop and implement individualized treatment plans focused on improving patient function and quality of life.

• Document all patient interactions and treatment progress accurately using Kinnser software.

• Communicate effectively with patients, families, and healthcare team members to coordinate care.

Requirements

• Travel to patient homes within Southfield and surrounding areas as required. REQUIREMENTS

• Valid physical therapy license in the state of Michigan.

• Experience with Kinnser homecare software required.

• Proven ability to work independently and manage time efficiently.
