About the Role

Join Olympia Fields Hospital as a Physical Therapist – Home Care Per Diem in United States. Responsibilities Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals. Performs patient treatment according to Physical Therapy procedures of modalities, therapeutic exercise, gait training and other accepted methods.

Key Responsibilities

• Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals.

• Performs patient treatment according to Physical Therapy procedures of modalities, therapeutic exercise, gait training and other accepted methods.

• Communicates pertinent information verbally or written to physician, other staff members, patients/caregivers.

• Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program.

Requirements

• EDUCATION, EXPERIENCE, TRAINING 1.

• Bachelor, Masters, or Doctorate Degree in Physical Therapy from an accredited program.

• One year recent clinical experience.

• Home Care experience preferred.
