Key Responsibilities

• Case Manager. You'll help deliver care in a holistic, family

• centered manner anticipating, preventing and treating undesirable symptoms or secondary symptoms.

Requirements

• Bachelor's Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing within the state of employment.

• Minimum 3 years inpatient clinical experience, preferably hospice.

• Valid driver's license and auto insurance in your name as a driver.

• Capable of all physical demands.

Work Schedule

• Full-Time position.

• Weekend coverage may be required.
