Key Responsibilities

Case Manager. You'll help deliver care in a holistic, family

centered manner anticipating, preventing and treating undesirable symptoms or secondary symptoms.

Requirements

Bachelor's Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing within the state of employment.

Minimum 3 years inpatient clinical experience, preferably hospice.

Valid driver's license and auto insurance in your name as a driver.

Capable of all physical demands.
