- Education: Masters Degree an accredited Nurse Practitioner program.
- License/ Certifications: Certification from an accredited Nurse Practitioner program.
- Valid Nurse Practitioner Furnishing number issued by the California Board of Registered Nurses.
- Valid California Registered Nurse/ Nurse Practitioner license.
