- Assesses residents’ needs and provides case management and support to promote and maintain an optimal quality of life and wellbeing.
- Within 30 days of admission obtain social history of new residents
- Assists with implementing Advance Directive if not currently on file.
- Coordinates individual service plans and assesses changes in residents’ psychosocial health needs.
