About the Role

Join PacificSource as a PACE Licensed Dietitian - Part Time in United States. Looking for a meaningful way to make a difference in the lives of older adults? At Pacific PACE LLC, you will be part of a team that provides compassionate, coordinated care that helps participants live safely, independently, and with dignity.

Key Responsibilities

  • Conduct comprehensive nutritional assessments of participants to identify their dietary needs and preferences across the continuum, including occasionally visiting home and community settings.
  • Actively participate in the IDT by offering clinical expertise in team planning, problem-solving, and processes.
  • Develop and implement individualized nutrition care plans to address participants’ health conditions and dietary goals.
  • Assess nutritional needs, diet restrictions, and current health plans to develop and implement dietary care plans and provide nutritional counseling.

Requirements

  • Serve on various internal and external committees as required or designated.
  • Participate in team meetings, caregiver groups, and special events as needed.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
  • Supporting Responsibilities: Meet department and company performance and attendance expectations.
