Registered Nurse | Care Coordination

About the Role

Join Akido as a Registered Nurse on the Enhanced Care Management team supporting Inland Empire Health Plan members in California. This role blends clinic-based, telehealth, and occasional community visits to coordinate care for patients with complex medical and social needs.

Key Responsibilities

• Provide RN-level care coordination for ECM-eligible members.
• Conduct nursing assessments via telehealth, telephone, clinic visits, and community/home visits.
• Perform medication reconciliation, health education, chronic disease monitoring.
• Develop and implement individualized care plans with interdisciplinary teams.
• Coordinate care across providers and community resources.
• Document encounters in compliance with ECM and HIPAA.

Requirements

• Valid California Driver's License and reliable transportation.
• Strong clinical decision-making and communication skills.
• Comfortable with telehealth platforms and electronic health records.
• Preferred bilingual English/Spanish and experience with Medi-Cal/Medicaid populations.
• Collaborative practice agreement required (supervision_required: true).
