About the Role

Join Akido as a Registered Nurse on the Enhanced Care Management team supporting Inland Empire Health Plan members in California. This role blends clinic-based, telehealth, and occasional community visits to coordinate care for patients with complex medical and social needs.

Key Responsibilities

Provide RN-level care coordination for ECM-eligible members.
Conduct nursing assessments via telehealth, telephone, clinic visits, and community/home visits.
Perform medication reconciliation, health education, chronic disease monitoring.
Develop and implement individualized care plans with interdisciplinary teams.
Coordinate care across providers and community resources.
Document encounters in compliance with ECM and HIPAA.

Requirements

Valid California Driver's License and reliable transportation.
Strong clinical decision-making and communication skills.
Comfortable with telehealth platforms and electronic health records.
Preferred bilingual English/Spanish and experience with Medi-Cal/Medicaid populations.
Collaborative practice agreement required (supervision_required: true).
