Key Responsibilities

Completes initial nutrition assessments and reassessments in accordance with departmental policies and guidelines.

Develops and implements evidence

based nutrition care plans tailored to patients medical conditions and needs.

Evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention and follows up as patient needs change.

Requirements

the credentialing Agency for Academy of Nutrition and Dietetics)

must be obtained within the first six months of employment. New York State Certified Dietitian Nutritionist (CDN)

must be obtained within 1 year of employment. Effective oral, written communication and interpersonal skills.. Required Licenses: Dietetics
