About the Role

Join Trinity Health as a Pharmacist - Northern Network in United States.

Key Responsibilities

Clinical Practice: Provide comprehensive medication management for chronic disease states such as diabetes, hypertension, hyperlipidemia, asthma/COPD, osteoporosis and heart failure.

Conduct patient assessments, develop care plans, and monitor therapeutic outcomes in collaboration with the healthcare team.

Manage anticoagulation therapy (e.
