About the Role
Join Trinity Hospice as a Hospice Social Worker in United States. Job Summary: The Social Worker will provide social services under the direction of a Physician, to offer support to our patients and their families. Social work services will be based on the patient’s psychosocial assessment, and the patient and family’s needs.

Requirements
- of the agency/licensing board.
- Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
- Promote Agency philosophy and administrative policies to ensure quality of care.
- Attend interdisciplinary team meetings, participate in the patient care planning process and collaborate with the Interdisciplinary Team to promote coordination of patient care.
Work Schedule
- Full-time position.
- Call responsibilities required.
