About the Role

Key Responsibilities

Deliver comprehensive NP level assessment and management for residents with dementia, including non

pharmacological and pharmacological strategies within scope and legislation.

Provide onsite clinical support to care teams, collaborate with GPs, and coordinate case conferences to ensure continuity of care across primary care and LHD

Build capability through targeted education and coaching for RNs, care staff, and families to strengthen dementia health literacy

Requirements

of 5 years of nursing experience, including at least 12 months in Aged Care.

with demonstrated expertise in dementia care and complex behaviours in residential aged care.

Skilled collaborator with primary care, allied health, and hospital outreach; confident coordinating case conferences and integrated care plans.

] Excellent communication and interpersonal skills, with the ability to lead, mentor, and inspire others in their professional development.
