About the Role

Key Responsibilities

  • Deliver comprehensive NP level assessment and management for residents with dementia, including non
  • pharmacological and pharmacological strategies within scope and legislation.
  • Provide onsite clinical support to care teams, collaborate with GPs, and coordinate case conferences to ensure continuity of care across primary care and LHD
  • Build capability through targeted education and coaching for RNs, care staff, and families to strengthen dementia health literacy

Requirements

  • of 5 years of nursing experience, including at least 12 months in Aged Care.
  • with demonstrated expertise in dementia care and complex behaviours in residential aged care.
  • Skilled collaborator with primary care, allied health, and hospital outreach; confident coordinating case conferences and integrated care plans.
  • ] Excellent communication and interpersonal skills, with the ability to lead, mentor, and inspire others in their professional development.
