Key Responsibilities

- Provide counseling and support to patients, utilizing evidence
- based techniques
- Monitor patient progress and make necessary adjustments to care plans based on ongoing assessments
Requirements
- Qualifications: To qualify you must have a New York State Social Work License (LMSW or LCSW) required, with 13 years of relevant experience or an equivalent combination of education and experience.
- The successful candidate must be able to communicate effectively with individuals at all levels of the center and the organization.
- Preferred Licenses: Licensed Social Worker - NYS Preferred Qualifications: LCSW preferred, with 35 years of experience in patient assessment, counseling, and care coordination.
- Qualified candidates must be able to effectively communicate with all levels of the organization.
