Key Responsibilities

Develop a psychosocial assessment to conduct upon intake with new patients

Provide counseling and support to patients, utilizing evidence

based techniques

Monitor patient progress and make necessary adjustments to care plans based on ongoing assessments

Requirements

Qualifications: To qualify you must have a New York State Social Work License (LMSW or LCSW) required, with 13 years of relevant experience or an equivalent combination of education and experience.

The successful candidate must be able to communicate effectively with individuals at all levels of the center and the organization.

Preferred Licenses: Licensed Social Worker - NYS Preferred Qualifications: LCSW preferred, with 35 years of experience in patient assessment, counseling, and care coordination.

Qualified candidates must be able to effectively communicate with all levels of the organization.
