Requirements

Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.

Licensed to practice as a physical therapist in the state of practice.

Must have a minimum of one (1) year experience as a physical therapist or have a preceptor for a minimum of 6

Current working knowledge of conditions of participation and home healthcare experience preferred.
