Key Responsibilities

• Patient population includes adult, adolescent, and geriatric patients depending on the practice location.

• Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.

• Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.

Requirements

• Pharm and equivalent combination of education and experience.

• Registered pharmacist in the State of Ohio; board certified preferred; advanced degree (Pharm.

• /MS) and residency; and/or equivalent experience preferred (e.

• PGY1 and PGY2 residency).
