Our busy, well-established primary care practice in the Farmington Valley is growing, and we’re excited to add a compassionate, team-oriented APRN to our group.

Key Responsibilities

• manage both acute and chronic conditions, and truly make a difference in your patients’ lives.

• Provide comprehensive primary care to adult patients Perform physical exams, diagnose and treat acute and ch.

• Develop and manage treatment plans Educate patients on preventive care and wellness Maintain accurate and ti.

Requirements

• licensed APRN (FNP or ANP) Primary care experience preferred, but strong new graduates are welcome Excellent communication skills A provider who enjoys teamwork and continuity of care
