Key Responsibilities

• This is a PRN position At CHI Health at Home, we strive to embody our mission of delivering hope and healing to those we serve.

• As a Medical Social Worker, you’ll engage in a range of exciting responsibilities, including: Establishing a care plan in accordance with the physician’s plan of treatment.

• Providing crisis intervention as required.

• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.

Requirements

• Master’s degree from an accredited school approved by the Council on Social Work Education.

• Currently licensed to practice in the state serviced.

• One year social work experience in a health care setting.

• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements Ready to Make a Difference.

Work Schedule

• Per Diem position.

• No explicit call, weekend, or night shift information provided.
