About the Role

Join Avalon Health Care as a Social Worker – Part Time or As Needed in United States. Brighten Life Home Health and Hospice in St. George, UT is looking to hire an experienced Social Worker for Part Time or as needed work.

Key Responsibilities

Develop a comprehensive social history assessment, a psycho

social assessment, and complete the MDS and Care Plan for new admits. Orient and assist new residents in adjusting to the services and resources that are available.

Assist residents and families with adjustment to changes, including level of care.

Document progress notes, which relate to each resident’s care plan, when necessary and within policy time

Requirements

MSW or LCSW required

Master’s Degree in Social Work or related field.

Health care experience preferred.

Must meet any state and/or Federal requirements if applicable.

Work Schedule

Part-Time position.

Day shift only - no nights.

Flexible scheduling available.
