Program/ Location: Community Based Health Clinic / Outpatient Program / Brockton Education/ Licensure: Master's degree required, license preferred.

Key Responsibilities

psychosocial assessments

Responsible for each assigned patients medical records. All records are to be professional, complete and thorough with entries made in a timely manner

Responsible for linking patients with aftercare services prior to discharge and aiding the patient and his/her family access outpatient services when appropriate

Attend all unit, facility meetings and case conferences as directed by the Clinical Director

Requirements

Masters Degree Required and/or professional license in related field, Licensed appreciated

Experience with Behavioral health

Salary: $70K - $80K

Signing bonus: $1.

15 days PTO.

Work Schedule

Full-Time position.

Hybrid position - combination of remote and on-site.

Flexible scheduling available.
