Key Responsibilities

• Assess and address factors such as socioeconomic status, housing stability, and cultural influences to optimize patient outcomes.
• Provide data-driven, condition-specific specialty visits to patients as needed
• Commute to assigned patients private residences to provide care
• Our providers are supported by an excellent staff of MD's, telephone triage, marketing professionals, IT, billing and scheduling personnel.
• Bloom providers serve as the primary care provider for patients and see patients based on medical necessity
• Bloom treats patients longitudinally through a holistic primary care model

• Master's degree Required

Work Schedule
