Key Responsibilities
- Report to work as scheduled, prepared to perform all job duties
- Participate in daily team huddles and assess critical psychosocial needs requiring immediate attention
- Schedule and conduct daily patient and family visits
- Complete initial psychosocial and bereavement assessments within five days of hospice admission and communicate findings to the interdisciplinary team
