About the Role

Join Empo Health as a part-time contracted Licensed Practical Nurse (LPN) focused on remote patient monitoring. This fully remote role supports diabetic patients by monitoring foot images and weight data to detect early signs of diabetic foot ulcers.

Key Responsibilities

Monitor patient data daily via Empo Remote Health Link portal.
Escalate critical findings to supervising physicians.
Conduct routine patient check-ins and adherence reminders.
Provide outreach for patient questions and device issues.
Collaborate with patients' physicians and Empo supervisors.

Requirements

Licensed Practical Nurse accredited in Virginia.
Experience with diabetic patients.
Strong communication skills in English.
Tech-savvy with digital tools proficiency.
