Requirements

M. D. or D. O. degree with completion of an accredited Family Medicine residency

Current license to practice medicine in the State of Wisconsin

Board Certification by the American Board of Family Medicine (ABFM) or American Osteopathic Board of Family Physicians (AOBFP)

DEA registration and Basic Life Support (BLS Certification)
