Key Responsibilities
- Conduct thorough assessments to evaluate clients mental health status, behavioral issues, and psychosocial n.
- develop individualized care plan based off assessment and tailored to both medical and psychosocial needs.
- coordinate and expedite the patient through the continuum of care management.
- Provide immediate crisis intervention, including de-escalation techniques, brief counseling, safety planning.
