- This role reviews physician referrals, patients’ subjective report, Physical Therapy evaluation and goals, and the medical record, to implement the Physical Therapy treatment plan accordingly.
- Contacts the Physical Therapist regarding any change in status or proposed treatment plan.
- Provides education to patient, family and/or caregiver regarding purpose of activity, use of equipment, home exercise and activity.
