About the Role

Join Nuclear Care Partners as a Nurse Navigator in United States. Nurse Navigator (RN) Livermore, CA 1:1 Patient Care! The Nurse Navigator is responsible for the day-to-day management, coordination and supervision of the related clinical activities within their assigned service area utilizing professional standards of care and agency policy and procedures.

Key Responsibilities

• Expected to routinely complete face to face patient visits and staff visits in home setting for all patients on case load.

• Build face to face rapport with patient and patient families in implementing and managing care being rendered.

• Maintains communication with Regional Clinical Director, supervisor, case managers, RN Authorization Specialists, lead and team nurses, non

• licensed staff, support staff and ancillary services.

Requirements

• Bachelor of Science in Nursing preferred.

• Current RN licensure in the state of CA

• A qualified candidate must possess a strong sense of professionalism, effective interpersonal skills and be a true team player.

• Previous case management or leadership role(s) which would include Interviewing, hiring, on

• Salary: $58 - $67/hr

Work Schedule

• Full-Time position.

• Weekend coverage may be required.
