About the Role

Join Nuclear Care Partners as a Nurse Navigator in United States. Nurse Navigator (RN) Livermore, CA 1:1 Patient Care! The Nurse Navigator is responsible for the day-to-day management, coordination and supervision of the related clinical activities within their assigned service area utilizing professional standards of care and agency policy and procedures.

Key Responsibilities

Expected to routinely complete face to face patient visits and staff visits in home setting for all patients on case load.

Build face to face rapport with patient and patient families in implementing and managing care being rendered.

Maintains communication with Regional Clinical Director, supervisor, case managers, RN Authorization Specialists, lead and team nurses, non

licensed staff, support staff and ancillary services.

Requirements

Bachelor of Science in Nursing preferred.

Current RN licensure in the state of CA

A qualified candidate must possess a strong sense of professionalism, effective interpersonal skills and be a true team player.
