About the Role
Join Good Samaritan Hospice Indiana as a Hospice Nurse Practitioner in United States.
Key Responsibilities
- Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.
- Establishes a plan of care for continued needs based on the client assessment.
- Reports changes in patient condition to appropriate personnel and the attending physician.
- Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers.
Requirements
- Graduate of an accredited school of nursing.
- Currently licensed to practice in the state serviced, with Masters in nursing.
- At least 1 year of nursing experience Must obtain and maintain current CPR Strong critical thinking, clinical expertise, case management, and a passion for patient care.
- Valid driver’s license and auto insurance coverage according to the CommonSpirit Health at Home policy.
