Key Responsibilities

• Perform preventative care screenings and medication reconciliation.
• Diagnose medical conditions and provide health coaching, referrals, and treatment.
• Engage members virtually, including assessment, diagnosis, and treatment.
• Outreach to chronically ill members and improve access to care.
• Cross-train into urgent care and virtual primary care service lines.

Requirements

• Graduate of an accredited PA program.
• Board Certification (NCCPA).
• Licensed in Georgia and one of Florida, Texas, or Ohio.
• Bilingual Spanish fluency (read, write, speak).
• 3+ years outpatient Family Medicine, Internal Medicine, or Primary Care experience.

• Salary range: $104,000 - $136,500 per year.
