Key Responsibilities
The Primary Care Physician will provide dedicated care to seniors, managing a small patient panel of up to 450 patients, focusing on those with 3-5 chronic diseases within a value-based care model. The role involves a Monday through Friday schedule with no nights or weekends, supported by a dedicated care team including an MA, Scheduler, and Referral Specialist.
