Key Responsibilities

Build strong relationships with patients and provide an exceptional care experience, with a focus on compassion and clinical excellence

Support your patients with their acute care, preventative care, and chronic disease management needs with clear education, shared decision making, and appropriate continuity

Apply your full scope to create thorough diagnoses and treatment plans which utilize our multidisciplinary team to support patients with all interconnected aspects of their health journey

Requirements

Patient focused and service minded approach to delivering excellent, comprehensive care

Empathetic, collaborative approach and ability to work effectively with a wide range of internal and external stakeholders

Commit to working at HealthOne for a minimum 4 days per week + 1 weekend day per month, with reliable scheduling

Licensed to practice as a Family Physician in Ontario and in good standing with the CPSO
