About the Role

This Clinical Pharmacist position with Advocate Health offers a full-time role with flexible in-person or remote work options. You will oversee pharmaceutical care, design medication therapy regimens, and collaborate with healthcare teams to ensure safe medication use.

Key Responsibilities

• Oversee pharmaceutical care and medication therapy regimens.
• Provide patient education on medication use and adverse effects.
• Collaborate with healthcare teams on formulary adherence and medication safety.
• Serve as preceptor and mentor pharmacy students and residents.

Requirements

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school or FPGEC certificate.
• Licensed or eligible for reciprocity in state of practice.
• Immunization and injectable certification as required.
• No experience required.
