[Remote] Clinical Pharmacist Practitioner (VISN 22 CRH – Primary Care)

About the Role

This is a remote Clinical Pharmacist Practitioner position with the Veterans Health Administration's VISN 22 Clinical Resource Hub. You will provide comprehensive medication management primarily via telehealth, working independently to optimize patient outcomes in primary care and specialty settings.

Key Responsibilities

• Manage chronic primary care disorders including diabetes, hypertension, dyslipidemia, and more.
• Provide gender-affirming hormone therapy where applicable.
• Collaborate with primary, specialty, and urgent care providers to develop medication management systems.
• Conduct comprehensive medication management via telehealth modalities.
• Perform medication reconciliation, lab monitoring, and adjust therapies as needed.
• Participate in team meetings and provide education and training to clinical staff.

Requirements

• U. S. Citizenship required.
• Graduate of an ACPE-accredited pharmacy program (BS Pharmacy or Pharm. D.).
• Full, current, unrestricted pharmacy license.
• Minimum 1 year experience equivalent to GS-11 level.
• Proficient in telehealth modalities.
