Requirements

• Current unencumbered Oregon License to Practice with Prescription Authority
• Current DEA Number
• Current Basic Life Support Certification
• Physician: Graduate of an accredited medical school. Must be board eligible/board certified in family practice or internal medicine.
• A criminal background check and pre-employment drug screen are required once a conditional offer is accepted.
• Must be board eligible/board certified in family practice or internal medicine
