Requirements
- Current unencumbered Oregon License to Practice with Prescription Authority
- Current DEA Number
- Current Basic Life Support Certification
- Physician: Graduate of an accredited medical school. Must be board eligible/board certified in family practice or internal medicine.
- A criminal background check and pre-employment drug screen are required once a conditional offer is accepted.
