About the Role
Join an established MLTC health plan as a Social Work Care Manager delivering remote case management focused on medical, social, psychological, physical, and spiritual needs. This full-time role supports members in collaboration with PCPs and caregivers.
Key Responsibilities
- Develop, implement, and monitor care plans.
- Provide case management addressing diverse member needs.
- Collaborate with interdisciplinary teams.
Requirements
- MSW, LMSW or LCSW license.
- 3-5 years clinical social work experience in long term care, managed care, home care, geriatrics, or hospice.
- Knowledge of Medicare and Medicaid regulations.

- Salary range $80,000 to $90,000 annually.
- $15,000 sign-on bonus for NY State LMSW or LCSW.
