Telehealth Registered Nurse - Case Manager

About the Role

Aptive Resources seeks a dedicated Registered Nurse Case Manager to support the Teleoncology Division at Veterans Health Services. This role focuses on patient-centered care coordination for veterans.

Key Responsibilities

• Coordinate and manage comprehensive care plans.
• Collaborate with interdisciplinary teams.
• Conduct clinical assessments and develop individualized care plans.
• Monitor patient progress and adjust plans.
• Educate patients and families.
• Facilitate communication among patients, families, and providers.
• Maintain accurate patient records per VA policies.

Requirements

• Graduation from accredited nursing school.
• Current and unrestricted RN licensure.
• BLS certification required; ACLS preferred.
• Minimum 2 years acute care experience preferred.
• Strong clinical assessment and teamwork skills.
