Key Responsibilities

• Travel to patients’ homes to provide individualized physical therapy services that promote recovery, mobility, and independence

• Conduct comprehensive evaluations to assess patient conditions and rehabilitation needs

• Develop and implement goal

• oriented treatment plans in collaboration with patients, caregivers, and the interdisciplinary care team

Requirements

• Unencumbered Florida Physical Therapy License(required)

• Valid driver’s license, auto insurance, and reliable transportation (required)

• Experience with OASIS documentation(preferred)

• CPR Certification(required)

Benefits & Compensation

• Salary: $120,640 - $145,600

• Signing bonus: $1.

• 15 days PTO.

• 401(k) with employer match.

Work Schedule

• Full-Time position.
