About the Role

BAYADA Home Health Care is hiring an experienced Weekend Home Health Physical Therapist to provide care to adult and geriatric patients in Milford, Medway, Franklin, and surrounding towns. This flexible part-time or full-time role focuses on skilled nursing and rehabilitative care.

Key Responsibilities

• Conduct home visits.
• Perform diagnostic tests including mobility, balance, strength, and safety assessments.
• Develop and implement individualized care plans.
• Collaborate with nursing and multidisciplinary teams.
• Educate clients and families.
• Document care management using tablet technology.

Requirements

• Current Physical Therapist license in Massachusetts.
• Minimum one year recent clinical experience.
