About the Role

Join HOME CARE PROVIDERS GROUP as a Physical Therapist (PT)- Stamford/Norwalk in United States.

Key Responsibilities

  • Develop and implement individualized Plans of Care
  • Supervise Physical Therapist Assistants (PTAs) and/or H.
  • Coordinate care with physicians, caregivers, and the interdisciplinary rehab team
  • Maintain accurate documenta.
  • Conduct supervisory visits and manage discharge planning
  • Provide in-service education and participate in t.

Requirements

  • Current Physical Therapist license in Connecticut
  • Minimum of 1 year PT experience; Home Health and OASIS experience preferred
  • Experience with WellSky (Kinnser) software preferred
  • Physical exam within the last 12 months
