About the Role

BAYADA Home Health Care seeks an experienced Physical Therapist for per diem weekend Start of Care home health visits in Anne Arundel County, Maryland. Home health and Medicare OASIS experience required.

Key Responsibilities

• Conduct home visits within designated territories.
• Perform diagnostic tests and measurements including mobility, strength, balance, and safety assessments.
• Implement wound care interventions.
• Develop and revise individualized care plans.
• Educate clients and families on rehabilitative care.
• Document care management using electronic tablets.

Requirements

• Current Maryland Physical Therapist license.
• Minimum one year recent clinical experience.
• Graduation from accredited PT program.
• Ability to work independently with strong organizational skills.
