Key Responsibilities

• will do: Provides care coordination and assists with discharge planning services to patients who are in need of ongoing behavioral health consults in the EEH emergency departments or behavioral health inpatient care following discharge from the emergency department.

• will assist patients and families in providing continuity of care after discharge and directs patients and patient families to same, as appropriate.

• provide care appropriate to the age of patients served.

Requirements

• of one year of experience in a behavioral health care setting, preferably acute care.

• Preferred Education, Skills and/or Experience: A minimum of one year of previous social work experience.

Benefits & Compensation

• Hourly rate: $30/hr.

Work Schedule

• Full-time position.

• Call responsibilities required.

• Weekend coverage may be required.
