About the Role

Join Forge Health as a Licensed Therapist providing outpatient mental health and substance use services in Philadelphia, PA. Deliver evidence-based care in person and via telehealth, focusing on addiction and mental health conditions.

Key Responsibilities

• Facilitate outpatient and intensive outpatient programs.
• Conduct individual and family therapy.
• Assess clients using DSM criteria.
• Complete initial assessments and treatment plans.
• Coordinate care with multidisciplinary teams.
• Provide crisis intervention as needed.

Requirements

• Valid unrestricted Pennsylvania license (LCSW, LSW, LAPC, LPC, or LMFT).
• Master's degree in Social Work or related field.
• Experience in IOP settings preferred.
