About the Role

Serve as a Remote Patient Monitoring Home Telehealth RN Care Coordinator for the VA Greater Los Angeles Healthcare System. Manage complex patients via telehealth.

Key Responsibilities

• Coordinate care using telehealth technologies.
• Perform clinical assessments and triage biometric data.
• Collaborate with interdisciplinary teams.
• Lead quality improvement initiatives.

Requirements

• Active RN license.
• Nursing degree from accredited program.
• 2-3 years RN experience preferred.
• Case management experience preferred.
