Key Responsibilities

• will support a panel of our complex members through individual and group therapy sessions, while also providing psychoeducation and support to patients' families.

• will help members navigate the health and social services systems in the San Francisco Bay Area, connecting patients to resources and the community.

• will initiate, establish and maintain relationships not only with patients, but also with families, as you care for members of Amae Health.

• lead stable, meaningful lives, while ending the cycle of repeat hospitalizations.

Benefits & Compensation

• Salary: $70K - $115K

• 401(k) with employer match.

Work Schedule

• Full-Time position.

• Weekend coverage may be required.

• Day shift only - no nights.
